一、體檢Physical Examination
1.國際學生必須在到校后一周之內到成都市桐梓林北路一號四川國際旅行衛生保健中心體檢；
1. Students should take the physical examination within one week after their arrival at SWPU at Sichuan International Travel Healthcare Center（Address: Tongzilin North Road No. 1, Chengdu, Sichuan）；
2.新生去體檢時，須準備以下材料：
（1）已在自己國家體檢過的國際學生，攜帶《外國人體格檢查記錄》，交四川國際旅行衛生保健中心確認；
（2）護照原件及復印件；
（3）近期2寸正面免冠白底照片兩張；
2. Students should take the following materials to the physical examination:
(1) Physical Examination Record for Foreigners if applied
(2) Passport and a copy of the passport
(3) Two recent 2-inch-sized color photos（full face must be clearly visible, no head-cover, with a white background）.
3.國際學生體檢費用自理，約500元人民幣。體檢須空腹，早上11：30后不進行檢查；
3.The physical examination fee of about 500 RMB shall be covered by students themselves. No breakfast before the physical examination and the examination shall be finished by 11:30am.
4.經檢查確認，患有我國法律規定不準入境的疾病者，或患有嚴重疾病者，學校不予接收，應當立即離境回國；
4. Those who are ascertained upon examination as patients of any disease that no entry is allowed, as specified in China’s laws, shall leave immediately for their home country.
5.新生在獲得《境外人員體檢檢查記錄驗證證明》后才能辦理居留許可；
5. Students can apply for the residence permit only after they obtain the“Certificate of Verification for Physical Examination Record for Foreigners or Overseas Chinese”issued by Sichuan Entry-Exit Inspection and Quarantine Bureau.

二、住宿登記Accommodation Registration
1.凡有出境記錄的和辦理簽證延期的國際學生在來校24小時內均須在新都城東派出所辦理住宿登記，領取住宿登記表。
1. Students who have travel abroad or need to apply for extension of the residence permit should go to register within 24 hours after their arrival at SWPU as an international resident and get the Accommodation Registration Form at the local police office.
2.所需材料：
（1）護照原件和復印件（復印件包括護照首頁、簽證頁、入境章頁）；
（2）2寸正面免冠白底照片兩張；
（3）在讀證明（國際合作與交流處提供）；
2. Material prepared for the accommodation registration:
(1) Passport and a copy of the passport（the first page, visa page and the latest entry stamp page）
(2) Two recent 2-inch-sized color photos（full face must be clearly visible, no head-cover, with a white background）
(3) Certification of the Student Status（you can get from the Office of International Affairs）
三、居留許可證Residence Permit Application
1.新生在入境30天內辦理完住宿登記表和體檢證明后到成都市公安局出入境管理中心辦理居留許可證，所需材料如下：
（1）護照原件和復印件（復印件包括護照首頁和簽證頁）
（2）住宿登記表
（3）體檢證明
（4）2寸正面免冠白底照片兩張
（5）JW202表
（6）學校證明（國際合作與交流處提供）
（7）學校聯系卡（國際合作與交流處提供）
1. New students must apply to the Exit and Entry Administration Bureau of Chengdu Public Security Bureau for a residence permit within 30 days after their entry in China.Required materials:
(1) Passport and a copy of the passport（the first page, visa page and the latest entry stamp page）
(2) Accommodation Registration Form
(3) Physical Examination Record
(4) Two recent 2-inch-sized color photos（full face must be clearly visible, no head-cover, with a white background）
(5) Form JW 202
(6) Student’s Identity Certification（you can get from the Office of International Affairs）
(7) A copy of the Contact certificate（you can get from the Office of International Affairs）
2.凡參加校外實習的國際學生，學生本人須提前三個月到國際合作與交流處報備并按時到成都市公安局出入境管理中心辦理居留許可實習加注。所需材料:
（1）護照原件和復印件（復印件包括護照首頁和簽證頁）
（2）住宿登記表
（3）2寸正面免冠白底照片兩張
（4）JW202表
（5）實習單位同意接收函
（6）校外實習學院同意函（相關學院提供）
（7）學校證明（國際合作與交流處提供）
（8）學校聯系卡（國際合作與交流處提供）
2. Students who take part in the internship off campus should submit application documents to Office of International Affairs three months in advance and go to the Exit and Entry Administration Bureau of Chengdu Public Security Bureau applying for residence remarks in time. Required materials:
(1) Passport and a copy of the passport (the first page, visa page and the latest entry stamp page)
(2) Accommodation Registration Form
(3) Two recent 2-inch-sized color photos (full face must be clearly visible, no head-cover, with a white background)
(4) Form JW 202
(5) Approval letter from internship organization
(6) Approval letter from students’school.
(7) Student’s Identity Certification (you can get from the Office of International Affairs)
(8) A copy of the Contact certificate (you can get from the Office of International Affairs)
四、簽證、居留許可證等注意事項Notes for Visa and Residence Permit
1.護照或復印件應隨身攜帶，警察隨機抽查并有權對無證件留學生進行處罰；
(1) Always bring with you a copy of your passport for the police officers’random checking. No ID certification will result in punishment by the police office.
2.妥善保管護照，以防丟失，如丟失應立即向國際合作與交流處、當地派出所、成都市公安局出入境管理局、駐華大使館報告，并同時申請補辦新護照；
(2) Keep your passport safe. In case of loss, please immediately inform the Office of International Affairs, local police station, Exit and Entry Administration Bureau and your Embassy in China, and begin to apply for a new one.
3.關注護照或簽證（居留許可）有效期，應及時辦理相關手續；
(3) Be alert to the expiration date of your residence permit, and apply for extension within the regulated time limit.
4.中國出入境管理法規定，簽證（居留許可）過期將受到處罰，嚴重者驅逐出境；
(4) According to the relevant Chinese laws issued by the Entry and Exit Administration Bureau of China, expired residence permit or visa will be deemed as illegal residence and thus leads to penalty which can be as serious as deportation.
5.在沒有登記的住所留宿屬于違法行為，將按照《中華人民共和國出入境管理法》進行處罰。
(5) Staying overnight in anywhere that is not registered is illegal and thus leads to penalty in accordance with the relevant Chinese laws.
