各學院：

近期國內多省出現確診病例，多地調整為中高風險地區，疫情形勢嚴峻復雜，防控壓力持續增大。為降低疫情輸入傳播風險，切實保障廣大學生的身體健康和生命安全，現就做好從“排查區域”（包括中高風險地區、低風險重點區域和出現本土病例未定風險區等，具體區域以屬地疾控部門實時更新發布的名單為準）返川、返校學生的摸排管理重申如下要求：

一、即日起，暫停受理學生因各類事由的暑期提前返校申請，2021年秋季開學返校安排另行通知。

二、要求各學院全面加強從“排查區域”返川、返校學生的摸排管理。

1．務必根據屬地疾控部門和學校發布的緊急排查指令和要求，對所有從“排查區域”返校的學生進行追蹤排查，排查結果及時交學校匯總后向屬地疾控部門報備。同時，要逐一了解其是否有48小時內核酸檢測陰性報告；沒有進行核酸監測的，應及時報告，由學校組織采樣檢測。

2．務必通知到所有學生，凡屬從“排查區域”返川的（不含返校），必須及時向所在地社區報備、出示48小時內核酸檢測陰性報告，堅決遵守所在地疫情防控管理規定，積極配合做好人員信息登記等相關調查，如實提供情況、準確反饋信息，主動納入社區管理，共同做好疫情風險排查管控工作。同時按照屬地要求，嚴格落實減少外出、不聚集、戴口罩等管理措施，如出現發熱、咳嗽等異常癥狀，及時到有發熱門診的醫療機構就診。

3．務必提醒到所有學生，對不如實報告有關情況、阻礙排查管理、拒不配合疫情防控措施落實的，將依法依規予以嚴肅處理，情節嚴重構成犯罪的，將移送司法機關依法追究法律責任。

三、切實壓緊防控主體責任。各學院要嚴格落實從“排查區域”返川、返校學生摸排管理的主體責任，配合做好信息摸排、情況報送和健康管理等工作，主動排查潛在薄弱環節和短板漏洞，落實落細各項管控措施。

Urgent Notice on Effectively Conduct the Investigation and Management of Students Returning to Sichuan and Campus from the "Investigation Area" for the Prevention and Control of the Pandemic

All schools:

Recently, confirmed cases have appeared in many provinces in China, and many places have been adjusted to medium or high-risk areas. The pandemic situation is severe and complicated, and the pressure of prevention and control continues to increase. In order to reduce the risk of import and spread of the pandemic, and effectively protect the health and life safety of all the students, now we reiterate the following requirements on effectively conduct the investigation and management of students returning to Sichuan and campus from the “Investigation Area” (including high-risk areas, medium-risk areas, low-risk key areas, undetermined risk areas with local cases, etc. The specific area is subject to the list updated and released in real time by the local disease control department):

I. With immediate effect, the acceptance of applications for early return to school during the summer vacation due to various reasons shall be suspended. Arrangements for the start of school and return to school in the fall of 2021 shall be notified separately.

II. All schools are required to comprehensively strengthen the investigation and management of students returning to Sichuan and campus from the "Investigation Area".

1. Be sure to follow the emergency investigation instructions and requirements issued by the local disease control department and the university to track and investigate all students returning from the "Investigation Area", and submit the investigation results to the university. The university will collect the data and report to the local disease control department. At the same time, it is necessary to know one by one whether there is a negative report of nucleic acid test within 48 hours; if there is no nucleic acid monitoring, it should be reported in time, and the university will organize sampling and testing.

2. All students must be notified. Students who return to Sichuan (excluding returning to campus) from the "Investigation Area" must promptly report to the local community, show a negative nucleic acid test report within 48 hours, resolutely abide by the local pandemic prevention and control regulations, actively cooperate with relevant investigations such as personnel information registration, provide truthful information and accurate feedback information, actively incorporate into community management, and work together to conduct pandemic risk investigation and control. At the same time, in accordance with local requirements, the student must strictly implement management measures such as reducing going out, not gathering and wearing masks. If abnormal symptoms such as fever or cough occur, the student should go to medical institutions with fever clinics in time.

3. It is important to remind all students that those who do not report the situation truthfully, obstruct investigation and management, or refuse to cooperate with the implementation of pandemic prevention and control measures shall be dealt with seriously in accordance with laws and regulations. If the circumstance is severe enough to constitute a crime, the violator shall be transferred to judicial authorities for legal responsibilities.

Ⅲ. Effectively tighten the responsibility of the main body of prevention and control. All schools must strictly implement the main responsibility for the investigation and management of students returning to Sichuan and campus from the "Investigation Area", cooperate with the work of information screening, situation reporting and health management, and proactively troubleshoot potential weaknesses and shortcomings, and implement various control measures in details.
